We have just increased our sign-on bonus for the month of May! This is
the highest Sign-On BONUS we have ever offered!

Requirements:

1. You must print this page and bring it with you when you register in order to qualify for the $2,000 Sign-On Bonus (RN's & LPN's Only).
2. Sign-On Bonus only applicable for hours worked at our hospital and Long Term Care clients.
3. Complete 150 billable hours by August 30, 2008 to receive your $2,000 BONUS. Orientations don’t count toward billable hours.
4. You must not cancel any shifts to be eligible for the $2,000 Bonus.
5. All applicants must visit one of our offices by May 31, 2008 to formally register for employment to qualify.
6. All applicants must bring all of the credentials listed below when registering for employment to qualify for the Sign-On Bonus. If you have any questions about your credentials please call us before arriving for your appointment so you can be assured that you will arrive with all the required credentials. You can also fax your credentials to us prior to your appointment if you prefer. We will immediately let you know if you are missing any items so you can get them prior to your appointment.

CREDENTIALS REQUIRED FOR HOSPITAL AGENCY NURSING:

1. RN licence
2. CPR card
3. ACLS card (for ICU, TELE, ER)
4. Annual TB
5. Chest X-ray (If your TB is Positive)
6. Annual physical (fit-for-work note from physician)
7. Mumps Titer with values or proof of a 2-shot booster
8. Rubella Titer with values or proof of a 2-shot booster
9. Rubeolla Titer with values or proof of a 2-shot booster
10. Varicella Titer with values or proof of a 2-shot booster
11. SS card or birth certificate or passport
12. Picture ID
13. N95 respirator fit test
14. Updated resume

CREDENTIALS NEEDED FOR LONG TERM CARE AGENCY NURSING:

1. Nursing License
2. CPR Card
3. Picture ID
4. SS Card or Birth Certificate or Passport
