Complete Online Application
This is a complete application that will serve as your formal application for Nursing Employment at Healthcare Personnel Inc.

The Process

1- Please start the process by filling out one of our online applications.

• The Short Online Application is a quick and easy way to start the process. I f you have a resume you can attach your resume to the Short Online Application . It is not required but it is very helpful for us to better understand your nursing background prior to contacting you to discuss nursing employment options.
• The Complete Online Application is a much more detail application that will serve as your formal application for Nursing Employment at Healthcare Personnel Inc.
  

  *You will notice that both of our online applications are secured using a technology called SSL encryption. This technology assures that the information you submit is protected and secure.

2- Once we receive your application, it is reviewed by our nursing recruiters. A nursing recruiter will then contact you by phone and/or email to follow up with your application. When we follow up with you we will do our best to answer all of your questions and together, talk about the types of nursing assignments that fit your background, experience, and interests.

3- After our initial discussions we will schedule an appointment for you to visit one of our offices to complete the application process. The application process takes approximately 1 hour to complete. We have two offices in the Chicagoland area for your conveniance:

The Formal Registration Process

The formal registration process includes:

1. Completion of an employment application
2. Various Tax forms
3. Skills Checklists specific to your nursing background.
4. Competency testing and evaluation
5. A personal interview
6. Facility specific orientations may be required depending on the type of assignments that fit your background and interests. We have an internal nurse trainer that will work with you to provide facility specific orientations.

Credential Requirements

Below is a list of the basic credential requirements that need to be met prior to starting any assignment at Healthcare Personnel. The credential requirements do very depending on the type of assignment. The list below is a guide. Please contact us to discuss your specific credential requirements.

1. Professional License RN, LPN, CNA, Other
2. CPR Card
3. ACLS Card (for ICU, Tele, ER)
4. Annual TB
5. Annual TB Questionnaire (if positive TB Test)
6. Chest X-ray (every 5 years, if positive TB Test)
7. Annual Physical
8. Mumps Titer (Hospital Staffing Only)
9. Rubella Titer (Hospital Staffing Only)
10. Rubeola Titer (Hospital Staffing Only)
11. Varicella Titer (Hospital Staffing Only)
12. Hep B or declination (Hospital Staffing Only)
13. SS Card or Birth Certificate or Passport
14. Picture ID

4- After the formal registration process it normally takes up too 3-5 days to process your application for employment.
